YAI seeks a START Coordinator to manage an active caseload (20–25) across START Services in New York. You will perform outreach visits (home, work, or community sites) via in-person or telehealth, conduct intake assessments, and coordinate with clinical leadership to ensure timely service delivery.
You will develop comprehensive service plans, participate in cross-system planning, and provide on-call crisis support as needed.
