Key Responsibilities- Perform thorough patient assessments and clinical reassessments

- Collaborate with an interdisciplinary care team
- Educate and support families/caregivers in ongoing home care
- Licensed Practical Nurse (LPN) with experience in skilled nursing
- Ability to manage high-acuity needs with strong clinical judgment
- Proficiency in medication administration, wound care, and trach/vent support
- Strong communication skills and patient/family education ability
- Accurate documentation skills in EMR systems
