Practice Nurse

Colton Mill and The Grange Medical Centre havean exciting opportunity for a Band 5 General Practice Nurse to join our friendly andhigh-performing team.

The post holder will be a registered health professional who holds a Degree Level Adult Nursing Qualification. This role will deliver high quality patient care to meet the holistic health care needs of a practice population. The post holder is responsible for the delivery of practice nursing services care to the population the practice serves and primary care network. Supported by senior nurses within the practice, they will deliver care within their scope of practice.

They will be focusing upon supporting patients to be healthy, monitoring long-term conditions, and engaging in health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

You will be joining a well-establishedmultidisciplinary team including: 9

Partners (8 GPs and 1 Managing Partner) 3GPs and 5 GP Registrars 2

Pharmacists, 3 experienced Practice Nurses, a Nurse Associate, and an HCA An experienced and friendly admin, reception, and management team We are proud to be part of the Cross Gates Primary Care Network (PCN), working collaboratively with neighbouringpractices to deliver a wider range of community-focused services.

Advanced Training Practice
As an Advanced Training Practice, we provide education and supervision for a wide variety of healthcare professionals including Student Nurses, Nurse Associates, GP Registrars, and Pharmacists. Allour staff are encouraged to take part in mentoring and supervision, and weparticularly welcome applicants who hold or are working towards a supervisorqualification to support our student nurses.
