• Conduct Care Act 2014 assessments for adults with cognitive and/or physical disabilities.
• Develop and review person-centred, strengths-based care and support plans to maximise independence.
• Apply the Mental Capacity Act 2005
  , completing capacity assessments and best interest decisions.
• Work collaboratively with advocates, families, carers, and multidisciplinary professionals
  .
• Identify and respond to safeguarding concerns
  , completing risk assessments and protection plans.
• Liaise with health professionals, occupational therapists, and voluntary sector partners
  .
• Support transitions from children’s to adult services
  , ensuring continuity of care.
• Maintain accurate, timely, and high-quality case records in line with Social Work England standards and local policies.
• Engage in supervision, training, and continuous professional development
