Gentiva Hospice seeks a dedicated RN Case Manager to provide in-home nursing care, assessments, and care coordination for patients with life-limiting illnesses. You will collaborate with physicians, social workers, and other team members to execute personalized care plans and ensure supportive end-of-life care.
You will educate patients and families, document thoroughly, and participate in on-call activities as needed. A valid RN license and CPR certification are required.
