Job summary

An exciting opportunity has arisen for a suitably experienced Occupational Therapist to join the Stroke Therapy Teams at Liverpool University Hospital Foundation Trust. Our centralised and world-leading North Mersey stroke centre will offer the post holder stroke specific rotations between two hospital sites (Aintree and Broadgreen). These rotations will include the Hyperacute stroke unit, Acute/Rehab stroke units and our Early Supported Discharge community teams.

Your role will be to support the rehabilitation and seamless transition of patients across the Stroke pathway.

This post requires excellent interpersonal and communication skills and a commitment to effective team working and skill sharing. The post holder will have experience working with adults with complex presentations, in particular Stroke and neurological conditions. The successful candidate will demonstrate leadership qualities and be able to evidence supporting staff/student development. There will be opportunity to contribute to service developments that are both locally and nationally driven.

You will receive regular supervision/appraisal and have access to specialist training and learning opportunities in order to support your career development in this specialist field.

In order to fulfil the Early Supported Discharge element to this post, access to a vehicle with appropriate business cover is advantageous.

Main duties of the job

The post holder will have the opportunity to work closely with highly skilled multidisciplinary colleagues, your named supervisor and support of the Advanced Clinical Practitioner and Therapy Management Team.

Responsibilities
  • Carry out neurological therapy assessment to identify post‑stroke deficits and clinically reason the need for further assessment and treatment.
  • Identify the most appropriate form of ongoing assessment including functional assessments, seating, upper limb and need for splinting along with standardised cognitive and mood screens.
  • Set patient‑centred goals, devise and regularly review treatment plans to achieve these.
  • Work closely with the MDT to identify rehabilitation potential and the most appropriate setting for this to occur (e.g., stroke rehab unit or own home).
  • Communicate effectively with patient, family and wider organisations to support patient recovery and coordinate a safe transfer of care/discharge.
  • Effectively manage own caseload, prioritise and delegate to generic therapy assistants.
  • Identify all necessary equipment to facilitate discharge and complete timely referrals to equipment supplier.
  • Complete clear and concise referrals to other services and coordinate a timely discharge to ensure maintenance of patient flow.
  • Ensure compliance with mandatory training and that all trust policies are followed.
