A leading GP practice in Fazakerly is seeking an experienced and proactive Practice Nurse to join their dynamic team. This is an excellent opportunity for a nurse with strong clinical skills and a passion for patient care to make a real impact in a supportive, forward-thinking practice.

Hours:

Full Time/ Part Time Salary:
Competitive, dependent on experience

• Deliver high-quality patient care including long-term condition management (Diabetes, Asthma, COPD, CHD, HTN, AF, CKD).
• Perform immunisations, vaccinations, and travel health advice.
• Conduct wound care, ECGs, venepuncture, and health assessments.
• Provide telephone triage, face-to-face consultations, and autonomous clinical decision-making.
• Contribute to safeguarding, infection control, and CQC compliance initiatives.
• Collaborate closely with GPs, ANPs, physiotherapists, and wider MDT.

• Registered Nurse (NMC) with valid PIN.
• Proven Practice Nurse experience in GP surgeries or primary care settings.
• Strong clinical skills and confidence in autonomous patient assessment.
• Excellent communication and team-working abilities.
• Experience with mental health, learning disability, or chronic disease management is desirable.
