Our vision is for all adults living in the Royal Borough of Greenwich to live well. We’re continuously striving to provide better outcomes for Greenwich residents. This is an exciting opportunity to join the CRSTAT (Community Rehabilitation short‑term assessment team).
We’re looking for a qualified, experienced Health and Adults Social Worker (part‑time 17.5 hours per week) with substantial knowledge of adult social care legislation, principles and best practice. You will also need to be currently registered with Social Work England to join us.
When working creatively within the CRSTAT using the Strengths‑based and Person‑Centred Approach, you will help residents experience personalised, integrated care. You will also support residents in your professional practice to deliver a high‑quality, robust, efficient and effective rehabilitation service that maximises individual user outcomes and enables independence in collaboration with family/carers and partnership organisations. You will support residents to maintain their independence and well‑being through timely assessments of their needs, implementation and review of packages of care, completion of carer assessments, and collaboration with health and social partners to manage change, achieve desired outcomes, understand and manage risks in relation to social inclusion, and facilitate and review care home placements when required.
You have experience of assessing the needs of adults and managing risks, including safeguarding enquiries.
You’ll report to the Team Manager and contribute towards the improvement and efficiency of the team.
