Working in line with individual care and reablement plans, you will provide personal, practical and rehabilitative support to help people maximise their independence in their own homes. This will include supporting individuals with daily living tasks, personal care where required, and encouraging skills development that promotes long‑term independence rather than ongoing reliance on support.

You will be responsible for observing, recording and reporting any changes in an individual’s condition or circumstances, ensuring concerns are escalated promptly to senior staff in line with policies, procedures and safeguarding responsibilities. This helps to ensure the ongoing health, wellbeing and safety of the people you support.

The role also involves researching and providing information about community and national resources, including social activities, benefits advice, local services, and cultural or religious support groups. You will signpost individuals to appropriate services to help them stay connected and supported within their community.

You will also be expected to comply with local policies and national standards, participate in training and appraisal processes, and contribute to the continuous improvement of both personal practice and service delivery.

This is a rewarding role where you can make a real difference by supporting people to rebuild skills, confidence and independence in their everyday lives.
