Job summary

We are currently looking for dynamic individuals to provide trauma informed care co-ordination to a complex caseload of patients requiring specialist forensic support as part of a multi-disciplinary team. To provide physical, psychological and emotional safety for our patients to empower individuals to re-establish control of their lives.

We are looking for individuals with core transferable skills from nursing, social worker or allied health background, we aim to provide staff with development in advanced risk assessment techniques as well as psychological approaches to patient care to promote recovery and sustainable life skills within the forensic population.

Please note applicants will be required to pay for their DBS check. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Responsibilities
  • Act as designated care coordinator to provide specialist assessment and access to interventions for a diverse patient group and their families under the care programme approach.
  • Recognise when an individual requires increased levels of support to promote a sustainable lifestyle in the community.
  • Promote recovery and support patients to understand their own risks and work towards greater independence, utilising partner agencies and third sector organisations.
  • Demonstrate active clinical and managerial leadership skills within the team, providing education and supervision to junior staff members and other professionals.
  • Design, implement and evaluate safe, creative and effective treatment plans in collaboration with patients, carers and other professionals.
  • Maintain accurate, timely, and relevant clinical records.
  • Manage safe monitoring, administration of medication as required by Trust policy.
  • Apply relevant legislation including the Mental Health Act, Mental Capacity Act and Safeguarding.
  • Work with case managers, inpatient services, social care and third sector organisations to identify pathways and necessary living arrangements for sustainable community living.
  • Provide and deliver physical health care interventions and develop this role within the team.
  • Lead professional leadership within a multi-disciplinary team.
  • Comply with best practice directions (NICE) and participate in quality and safety initiatives.
  • Access and provide clinical supervision to maintain competency and deliver best quality patient care.
  • Undertake any other duties that would be a reasonable expectation of the role.
Qualifications and Requirements
  • Professional qualification in either nursing, social work or allied health profession and current registration with a professional body.
  • Full UK driving licence and vehicle for business use.
  • Demonstrated clinical expertise for banding and specialist area specific experience.
  • Willingness to understand new clinical practice, education and training that enhance service delivery.
  • Continued professional development to maintain professional clinical knowledge.
Benefits and Working Conditions

Band 6, salary £38,682 to £46,580 a year pro rata for part time, permanent contract, full time location Heatherdene, Mansfield NG19 7AE
