Additional full time Social Workers within the Preparing for Adulthood Team (PFA).

ResponsibilitiesYou will work in a strength-based way and aim to make a real difference to the lives of young people experiencing learning disabilities, physical disabilities, mental health and ASD. You will:
- Provide assessment and commissioning support for young people as they enter adulthood.
- Work with a multi‑specialist team and in accordance with legislation such as the Care Act, Mental Capacity Act and Mental Health Act.
- Support young people in achieving their aspirations and outcomes at a pivotal time in their life.
- Collaborate with Children’s Social Care, education and health to support transitions to adult services.
- Maintain safeguarding protocols and work within multi‑agency procedures.
- Social Work Qualification and registration by Social Work England.
- Good communication, time management and IT skills.
- Knowledge and experience of safeguarding adults.
- Full driving licence and access to transport (or suitable arrangements for disabled employees).
- Experience in an adult social work or therapeutic service is desirable.
