The Opportunity

The Field Nurse Case Manager (FNCM) is part of the Patient Services team, coordinating and delivering services related to the patient support programs. The FNCM manages all aspects of program support and may act as the primary contact for assigned health care providers, ensuring patients have easy access to the program and their therapies. The role includes assisting the prescribing physician with patient enrolment, including on-site visits to gather necessary information for forms in a concise, organized, and professional manner.

Day in the Life
  • Perform HCP and patient registration activities according to program protocols and procedures
  • Process data entry for information collected during program delivery
  • Respond to general inquiries by phone or web platforms about services, disease state or product information
  • Provide medical support to patients and HCPs, including education on diseases and therapies
  • Participate in training sessions on therapies provided by the pharmaceutical team
  • Provide telephone support, including maintenance and adherence support as needed
  • Administer and follow case-management protocols, including clinical monitoring, data collection, patient follow-up, compliance, and adverse event reporting
  • Work with the reimbursement specialist and physician to secure funding for therapies and assist with alternative funding when needed
  • Attend patient meetings to educate patients and providers about disease/product information and case management services
  • Identify and report adverse events to Health Canada and Med Info and document in source and third-party systems
  • Maintain confidentiality of client and company information
  • Liaise with physicians’ offices, pharmacies, infusion service providers to deliver coordinated service and resolve issues
  • Participate in ongoing continuing education and develop training materials for new PSP team members
  • Adhere to Sentrex Health Solutions policies and procedures
  • Enroll patients into the program and complete required follow-ups
  • Proactively support patients and HCPs, including enrollment, renewals, and reimbursement facilitation
  • Follow program protocols to ensure continuity of service delivery
  • Travel to physician offices as requested
  • Other duties as assigned by management
What you need to be set up for success
  • Must be a Registered Nurse in good standing with the provincial College of Nurses
  • 2–4 years of clinical experience
  • Experience in a Patient Support Program is an asset
  • Proven ability to work in a team environment
  • Ability to resolve conflicts professionally
