Advanced Nurse/Clinical Practitioner - Frailty
To act as line manager for the Frailty Nurse Practitioner and to take the lead in providing and maintaining a flexible clinical service for our older demographic in the community with an emphasis on care delivered at home.
This is a part time role working 27 hours per week.
Main duties of the jobHome Visits
- Conduct comprehensive nursing assessments for older and housebound patients.
- Monitor and assist in the management of chronic conditions within your competencies.
- Provide wound and catheter care assessment and care within your competencies.
Nursing/Care Home Case Management
- Act as a liaison between the GP surgery and care home staff.
- Undertake reviews of residents health and care plans.
- Support and record end-of-life care planning and advanced care discussions.
- Ensure compliance with safeguarding and best practice standards.
- Deliver seasonal vaccinations and other immunisations where required.
- Promote health education and self‑care strategies for older patients.
Multidisciplinary Team (MDT) Meetings
- Attend and contribute to MDT discussions on frailty.
- Collaborate with social care, community nursing, and allied health professionals.
In-House Acute Older Person Clinics
- Support same-day assessments for acutely unwell older patients within the practice.
- Participate in service development and quality improvement initiatives.
- Maintain accurate clinical documentation and adhere to confidentiality standards.
- Assist the practice in the meeting of QOF and similar activity‑based NHS targets.
- Undertake any additional roles agreed with the Partners, aligned with your training and competencies.
- Act as line manager for Frailty Nurse Practitioner.
You will be fully supported by and work alongside a fulltime Nurse Practitioner and assigned Visiting GP along with the broad clinical team at the practice comprising of GPs, Clinical Pharmacists, Paramedics, Practice Nurses, Advanced Clinical Practitioner and Health Care Assistants. Experience with older patient care essential.
- Experience of practice within the four pillars
- Experience of prescribing and undertaking medication reviews
- Experience of working in a primary care environment
- Experience of working as a practice nurse or community nurse
- Registered Nurse with Nursing and Midwifery Council
- Masters Degree OR Postgraduate diploma or degree for Advanced Practice Qualification OR RCN accreditation
- Meets NMC revalidation requirements
- Meets the standards for registered ANP working at advanced level
