The General Medical Practitioner (GMP) will provide high-quality, patient-centred care to individuals and families within the Primary Care Network. The GMP willbe responsible for diagnosing and treating a range of medical conditions,promoting health and well-being, and working collaboratively with other healthcare professionals to enhance patient outcomes.
Main duties of the job- Conduct comprehensive assessments and examinations of patients.
- Diagnose and manage acute and chronic medical conditions.
- Develop and implement individualised care plans in collaboration with patients and the healthcare team.
- Prescribe medications and therapies as appropriate.
- Provide preventive care and health education to patients.
- Participate in multidisciplinary team meetings and case discussions.
- Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
- Engage in continuous professional development and training.
- Support the PCN in achieving health targets and initiatives.
Nexus MK PCN offices are based in Linford Wood Milton Keynes and cover 5 practices which include Oakridge Park Medical Centre, Wolverton Health Centre, The Stonedean Practice, Sovereign Health Centre, Purbeck Medical Centre.

- Familiarity with electronic health record systems.
- Knowledge of current NHS policies and guidelines.
