Main duties of the job

Provide high-quality, evidence-based care to patients with diabetes, including Type 1, Type 2, and pre-diabetes, in line with national and local guidelines.

Undertake regular diabetes reviews, including monitoring HbA1c, blood pressure, lipids, weight, and lifestyle factors.

Support medication management, including insulin initiation and titration where appropriate, and promote safe use of medicines.

Deliver structured patient education on diabetes self-management, diet, exercise, and prevention of complications.

Assess, manage, and monitor diabetic foot health, including wound assessment and ongoing wound care management.

Providing general wound care services for patients with acute and chronic wounds, including leg ulcers and pressure areas.

Identify, manage, and refer patients with diabetes-related complications to appropriate services.

Maintain accurate, timely clinical records using the practice clinical system.

Work collaboratively with GPs, practice nurses, healthcare assistants, and the wider multidisciplinary team.

Participate in clinical audits, quality improvement activities, and service development.

Ensure compliance with infection control, safeguarding, and health and safety policies.

Our practice is led by a committed and experienced team of six GP Partners
, supported by one Salaried GP
. Together they offer high-quality, continuous, patient-focused medical care, leadership, service development, and mentoring.

Our nursing team consists of 13 dedicated professionals delivering quality, patient-centred care across the practice.

The team includes experienced specialist nurses, a paramedic practitioner, treatment room nurse, nurse associates, healthcare assistants (HCAs), and a phlebotomist, ensuring comprehensive services.

The Minor Illness Team assesses and manages same-day and urgent presentations, improving access while supporting GP capacity.

We are supported by a skilled Reception, Administration, and Dispensary Team that ensures smooth service running.

Job responsibilities

To provide high-quality, evidence-based diabetes care within a GP surgery, supporting patients to manage their condition and reduce complications. The post holder will work autonomously and as part of the multidisciplinary primary care team to deliver comprehensive diabetes management, education, and ongoing support.

Key Responsibilities
  • Deliver structured diabetes reviews for patients with Type 1, Type 2, and pre-diabetes.
  • Monitor and interpret clinical indicators including HbA1c, blood pressure, lipids, BMI, and blood glucose readings.
  • Support medication management, including insulin initiation and titration.
  • Provide patient education on lifestyle management, diet, exercise, blood glucose monitoring, and sick-day rules.
  • Identify early signs of diabetes-related complications and initiate timely referrals to specialist services.
Foot Health and Wound Care
  • Undertake diabetic foot assessments and provide ongoing monitoring.
  • Assess, plan, and deliver wound care for diabetic and non-diabetic wounds, including ulcers and chronic wounds.
  • Escalate concerns appropriately in line with local pathways.
Health Promotion and Screening
  • Deliver preventative care and health promotion activities related to diabetes management.
Multidisciplinary Working
  • Work collaboratively with GPs, practice nurses, healthcare assistants, pharmacists, and the wider MDT.
  • Participate in case discussions and long-term condition management clinics.
  • Contribute to service development and improvement initiatives, continuously working toward QOF targets.
Professional Responsibilities
  • Maintain accurate, clear, and timely clinical records using the practice clinical system.
  • Ensure care is delivered in line with NICE guidelines, local policies, and best practice.
  • Participate in clinical audit, quality improvement, and revalidation activities.
  • Maintain professional registration and undertake continuing professional development.
Governance and Safety
  • Adhere to infection prevention and control policies.
  • Follow safeguarding policies and procedures.
  • Contribute to maintaining a safe clinical environment.
Person Specification Qualifications
  • Qualification and experience in Diabetes.
  • Primary care experience.
  • Non-medical prescriber.
  • Competent in immunisations, smear sampling and wound management.
