About the Role

EAST:

MK Primary Care Network has an exciting opportunity for an advanced nurse practitioner to join our practice team. The role is offered on a full‑time basis, 37.5 hours per week over 5 days per week, between Monday and Saturday, on a rotation basis across the four surgeries – Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice, and The Grove Surgery – that make up the PCN.

The advanced nurse practitioner will act as an autonomous advanced practitioner, able to work independently as well as in conjunction with other members of the clinical team. The post holder will be an experienced, caring nurse who demonstrates highly developed clinical skills and critical thinking, while working within professional boundaries.

The advanced nurse practitioner will demonstrate safe, competent clinical decision‑making and deliver expert care, including assessment, diagnostic skills, treatment, and evaluation of patients who wish to be seen by a healthcare professional, presenting with minor illness and/or their continuing medical and nursing needs.

Responsibilities

Responsibilities of the role will include:

  • Acting as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond conventional nursing roles.
  • Working as an autonomous clinical practitioner/nurse, providing high‑quality and timely patient‑centred care.
  • Independently prescribing, working within current guidelines, reviewing, and monitoring patient medications in accordance with the formulary.
  • Delivering face‑to‑face/telephone/e‑consult/video consultations as well as home visits.
  • Directing care appropriately and managing complete episodes of care independently.
  • Undertaking consultations with patients of all ages, including children, as an autonomous practitioner and using own clinical judgement, diagnosing, treating, referring and/or discharging patients.
  • Working within and across the MDT teams and signposting/drawing upon the expertise of all members to support and meet the patients' needs and best interests.
  • Recording clear and contemporaneous consultation notes to agreed standards, ensuring accurate read coding.
  • Participating in, and contributing to, all clinical targets (QOF, QIS, ES etc).
  • Recommending, instigating, and explaining appropriate invasive and non‑invasive diagnostic tests, investigations and interpreting findings and reports.

Essential qualifications, skills, and experience
  • A registered nurse with the Nursing and Midwifery Council (NMC).
  • A Master’s degree from the Centre for Advancing Practice accredited MSc advanced practice programme or completion of the centre’s e‑Portfolio (supported) route; both pathways enable eligibility for an ‘Advanced’ digital badge issued by the centre, demonstrating recognised educational and experiential preparation in advanced practice.
  • A qualified independent nurse prescriber, on the NMC register.
  • Meet NMC revalidation requirements, in accordance with the NMC revalidation booklet.
  • Meets the standards for registered ANP working at advanced level.
  • The ability to record accurate clinical notes.
  • The ability to work within own scope of practice and understand when to refer to a GP.
  • Experience of working in a management role and evidence of leadership skills.
  • Experience of working in a wide range of primary care settings, including clinical triage experience.
  • The ability to problem‑solve and to process information accurately and effectively, interpreting data as required.
  • Knowledge of health promotion strategies.
  • Understand the requirement for PGDs and associated policy.
  • Polite and confident, flexible and cooperative.
