This Practice Nurse role will work primarily across the treatment room three days per week and long‑term conditions clinics two days per week. Experience in Infection Prevention and Control (IPC) is essential. The post holder will play an active role in supporting and strengthening IPC arrangements within the practice, working closely alongside the Advanced Nurse Practitioner in relation to infection control governance, audits, evidence gathering, and ongoing compliance with CQC and NHS requirements.

This role is for an experienced Practice Nurse delivering the full range of treatment room nursing services alongside long‑term conditions clinics. The post holder will provide high‑quality, evidence‑based care including wound care, phlebotomy, ECGs, new patient checks, childhood and adult immunisations, and cervical cytology, all of which are essential to the role.

The nurse will also undertake structured reviews for patients with long‑term conditions such as diabetes, asthma, hypertension, and COPD, working autonomously within their scope of practice and contributing to safe, effective care pathways.

A key aspect of the role is active involvement in infection prevention and control (IPC). The post holder will support IPC activity through audits, compliance monitoring, and day‑to‑day best practice, working closely with the Advanced Nurse Practitioner and wider clinical team.

The role requires confident clinical decision‑making, excellent communication skills, and a strong commitment to clinical governance, patient safety, and high‑quality care within a multidisciplinary primary care team.

Job responsibilities

Job Title: Practice Nurse

Accountable to: Practice Manager (administrative), Nurse Manager (clinical)

Hours: As agreed (primarily treatment room and long‑term conditions clinics)

Job Summary: To be responsible for the implementation of processes for the effective management of patients with long‑term conditions using evidence‑based practice, including care for elderly and housebound patients.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well woman and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

The organisation will ensure the post holder has access to appropriate clinical supervision and a named individual to provide advice and support on a day‑to‑day basis.
