OverviewIn accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in (an) acute inpatient areas, and works collaboratively with the interprofessional care team to deliver person-centered care.
Responsibilities- Assess patients and collect information from a variety of sources using observation, communication, data review and physical assessment. Make a nursing diagnosis and determine whether the condition can be improved or resolved by an appropriate nursing intervention. Plan, perform and evaluate nursing interventions and care.
- Collaborate with patients and families/caregivers in developing the care plan; encourage family participation and ensure the patient’s rights to privacy, dignity and access to information, and support patient autonomy in decision making and care planning.
- Participate in care and discharge planning by ensuring the care plan is established and updated, and collaborate with the interprofessional care team and the Patient Care Coordinator on issues affecting care or discharge.
- Coordinate nursing care for the patient; set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols and practice standards.
- Collaborate with Physicians, Licensed Practical Nurses, Patient Care Coordinator, Clinical Nurse Educators and other health care professionals to identify and resolve issues, adjust care plans as required, and respond to changes in patient condition.
- Teach patients and families/caregivers about illness prevention, health restoration and health maintenance; assess learning needs and provide information and demonstrations to support self-care and the discharge plan.
- Discuss referral information with patients and families/caregivers; initiate referrals to appropriate health care/community agencies in collaboration with the interprofessional care team.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Participate on nursing committees, care conferences and councils as assigned; engage in research and quality improvement activities and contribute to changes in practices, procedures or protocols.
- Participate in in-services and other educational programs; identify learning goals and maintain current clinical competence within the area of practice.
- Act as a preceptor or mentor to assigned new nursing staff and students; contribute to orientation and share knowledge.
- Provide feedback to the Manager regarding the effectiveness of patient care programs, policy/clinical decision support tools and equipment.
- Performs other related duties as required.
Education and Experience
Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

- Ability to complete initial and ongoing patient assessment and provide nursing care through appropriate technical, therapeutic and safety interventions.
- Ability to communicate effectively with patients, families/caregivers, the public, medical staff and the interprofessional care team using verbal, written and electronic means.
- Ability to promote person-centered care with empathy, cultural sensitivity and patient advocacy.
- Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
- Ability to teach and instruct patients and families/caregivers about essential health topics.
- Ability to organize and prioritize work; collaborate with the interprofessional care team.
- Ability to apply evidence-based practice to nursing decisions and actions.
- Ability to operate related equipment including relevant computer applications.
- Physical ability to perform the duties of the position.
