Position: Reablement Officer - Ref: SS05025

Reablement Officer – Walsall Intermediate Care Service

Grade & Salary
Grade G4, £26,403–£28,142 pro rata to hours worked.

Working Hours
30 hours per week.

We are looking for enthusiastic and committed individuals with experience in adult social care to join our multidisciplinary team to support customers in their homes and deliver active reablement programmes.

Responsibilities

  • Support customers in developing or regaining skills and confidence, coordinating and delivering reablement programme to achieve full potential.
  • Support/enable activities of daily living (ADL’s) including personal care as part of an overall support plan, ensuring cultural and privacy needs are met.
  • Co‑coordinate and deliver reablement activities in partnership with other agencies, maintaining the agreed parameters within the support plan.

Qualifications & Experience

  • Experience working in community based reablement services.
  • Experience working with customers in their own homes.
  • Understanding of strength based working.
  • Excellent written and verbal communication skills.
  • Minimum qualification NVQ L2 Health and Social Care or equivalent.
