Diabetes Specialist Nurse

Don't miss out on this rare opportunity to join an established team of Highly Specialised Diabetes Nurses in our Mid-Nottinghamshire team. Our Diabetes Nurses work with primary care colleagues to provide the highest quality proactive care to our patients as part of the Diabetes LES (Local Enhanced Service) in Nottinghamshire.

You will work as part of a system to provide the best care possible in an innovative primary-care facing enhanced service, with training and development at the heart of its delivery.

You will work in predominantly clinic settings with the Newark PCN area to provide LES clinics with our primary care colleagues.

You will help develop the skills in primary care.

You will be supported by your peers and colleagues in secondary care to provide the highest possible level of care.

You will work with our community nursing colleagues and wider MDT to help ensure that patients in the community are optimised.

You will be a key part of a highly successful team.

• Work within the Diabetes LES (Local Enhanced Service) in Nottinghamshire.
• Provide proactive care to patients in Mid-Nottinghamshire.
• Deliver care in clinic settings with the Newark PCN area.
• Help develop skills in primary care.
• Collaborate with peers and secondary care colleagues to deliver highest level of care.
