Location: Newcastle upon Tyne
We are seeking a highly skilled and motivated Registered General Nurse (RGN) to join our team as a Complex Case Manager. This is an exciting opportunity for a dynamic individual with proven experience in complex case management within the community and a strong track record in business development.
As a Complex Case Manager, you will play a pivotal role in managing complex patient cases, ensuring the delivery of safe, effective, and personalised care. You will also take a proactive approach in developing new opportunities, building relationships, and supporting the growth of our services.
Key Responsibilities:- Lead and coordinate the management of complex patient cases in the community, ensuring high-quality, patient-centred care.
- Undertake comprehensive assessments and create tailored care plans to meet individual needs.
- Work collaboratively with patients, families, carers, clinicians, and multidisciplinary teams.
- Ensure adherence to clinical governance, safeguarding, and regulatory frameworks.
- Identify and develop new business opportunities, fostering strong partnerships with referrers, commissioners, and stakeholders.
- Provide expert clinical input to support service growth and innovation.
- Monitor and review outcomes to ensure continuous improvement in care delivery.
- RGN qualification with current NMC registration.
- Demonstrable experience in complex case management within the community setting.
- Strong business development experience, with the ability to build and maintain professional networks.
- Excellent communication, negotiation, and interpersonal skills.
- Ability to work independently and as part of a team, with strong organisational and problem-solving abilities.
- A flexible and proactive approach.
- Full UK driving licence and access to a car.
- Willingness to travel to our Manchester branch once per month.
- Salary up to £58,000 per annum.
- Mileage allowance of 40p per mile.
