We are looking for an ANP to join our friendly and supportive team at Newport Health Centre, based on the Isle of Wight.
This is a fantastic opportunity for an ANP to work alongside a supportive multidisciplinary team that includes Nursing, Allied Health Professionals and GPs, who have a real commitment to continuous improvement, shared learning and best practice, while being passionate about providing patients with the highest possible quality care.
Main duties of the jobThe Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits. They will also provide support on our ‘Chat Doc’ Whats App service (which has replaced our e-consult service).
You will be a core member of our innovative Acute Hub which provides on the day telephone or face to face appointments, working in a team of lead GPs, ANPs, Paramedics and Physician Associates.
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The postholder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.
The ANP will treat children and adults with a wide range of acute & minor ailments and common infections including, but not limited to
- Respiratory problems
- Rashes
- Urinary tract infections
- Ear, nose and throat problems
- Abdominal pain
- Lumps and bumps and recent back pain
Newport Health Centre is based centrally on the Isle of Wight operating across two sites; situated at our Carisbrooke practice and our town centre practice, Dower House, Pyle Street.
Our practice supports circa 24,500 patients on the Isle of Wight where we have a diverse, multi‑disciplinary team which includes GP Partners, Advanced Nurse Practitioners, Paramedics, Physician Associates, MSK Practitioners, Mental Health Practitioners, Practice Nurses, HCAs, Chronic Pain Coach, Care Coordinators, a Social Prescriber and many more vital roles to support our community. We are also an active part of the Central and West Primary Care Network (PCN).
Jobdescription Job responsibilities Clinical practice
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and seek appropriate advice from GP colleagues if necessary
- Provide general and specific health screening and proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non‑drug-based treatment methods into a management plan
- In conjunction with the doctors, prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
- Work with patients in order to support compliance with and adherence to prescribed treatments
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex situations, including initiation of effective care
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self‑care
