Practice Nurse Medical Practice

Overview

We are seeking an experienced, motivated and skilled Practice Nurse to join our supportive multidisciplinary team. The role involves working as an autonomous practitioner, delivering evidence‑based nursing care to the practice population. The candidate must be an NMC Registered Nurse with a recognised Practice Nurse qualification (degree level).

• Chronic disease management (e.g. diabetes, asthma, COPD)
• Assessment and treatment of minor illness, minor ailments and injuries
• Clinical assessment, diagnosis, treatment and evaluation of care
• Preventative healthcare and health promotion
• Safe clinical decision‑making and contribution to policy implementation
• Collaborative working within the general practice team

• Recognised Practice Nurse qualification (degree level)
• Experience in chronic disease management and minor illness
• Ability to work autonomously within a multidisciplinary team
• Experience working within a nursing team

• Independent Prescribing qualification
• Experience in primary care or community care
