District Direct Visiting Officer

Job Responsibilities

• Taking referrals from District Direct Officers and conducting domiciliary assessments.
• Liaising with a range of contractors to ensure homes are fit for discharge.
• Working as part of the Discharge Hub, visiting patients' properties and feeding back as to works required to facilitate discharge.
• Maintaining clear and accurate records of your interactions.

• Must hold a full driving licence and have access to a vehicle; travel is essential for this role.
• Experience working with people in distress.
• Solid understanding of common health problems.
• Excellent verbal and written communication skills.
• Ability to build and maintain professional relationships.
• Personal resilience and a good sense of humour.
