Job Responsibilities
- Taking referrals from District Direct Officers and conducting domiciliary assessments.
- Liaising with a range of contractors to ensure homes are fit for discharge.
- Working as part of the Discharge Hub, visiting patients' properties and feeding back as to works required to facilitate discharge.
- Maintaining clear and accurate records of your interactions.
- Must hold a full driving licence and have access to a vehicle; travel is essential for this role.
- Experience working with people in distress.
- Solid understanding of common health problems.
- Excellent verbal and written communication skills.
- Ability to build and maintain professional relationships.
- Personal resilience and a good sense of humour.
