Amedisys, part of the Optum family of businesses, is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating discharge and transition planning, and supporting ongoing outpatient care at home.
Under supervision of an RN or MD, you will educate, coach, and assist with treatment decisions while collaborating with a Care Advocate and Social Worker to ensure appropriate care levels.
