As our sole Practice Nurse, you will play a vital role in the clinical team.

Key Responsibilities:

• Management of Long-Term Conditions (supporting QOF targets, Spirometry and joint Diabetes Clinics)
• Health Promotion and Disease Prevention
• Childhood immunisations and Vaccination programmes
• Cervical screening
• Family Planning and Sexual Health
• Collaboration with Healthcare Teams
• Mentoring and Training (supporting the development of nurse students and junior staff, educational opportunities in clinical meetings within the practice)
• Involvement in the Patient Participation Group (PPG) and wider community health initiatives

We are a friendly, team-oriented and patient-centred GP practice serving a list size of approximately 5,000 patients in Sherwood. We are three GP partners and one ANP, each with more than 20 years experience, one HCA, admin staff, one PCN pharmacist and an online pharmacy team, a PCN mental health practitioner, and numerous auxiliary teams. As our current practice nurse is soon starting a new chapter in her life, this vacancy has become available.
