Are you an experienced Nurse Case Manager RGN seeking a rewarding, community-based role? Join one of the UK's leading complex-care providers as a Nurse Case Manager RGN, covering Nottingham and surrounding areas.

As a Nurse Case Manager RGN, you'll coordinate complex-care packages for adults and children, using your clinical expertise to assess, plan, and manage person-centred care in home and community settings. This hybrid position offers regional travel, autonomy, and genuine career progression.

• Manage a caseload of community complex-care clients across the Coventry region, ensuring safe and effective delivery.
• Carry out clinical assessments, risk reviews, and care-plan updates in line with best practice and CQC standards.
• Provide clinical supervision, mentorship, and competency sign-offs for carers and support staff.
• Conduct clinical audits, medication checks, and quality reviews to maintain compliance and governance.
• Liaise with families, ICBs, case managers, and MDT professionals to coordinate holistic care.
• Deliver training and clinical support to teams during both routine and emergency situations.
• Drive continuous improvement and uphold professional nursing standards as a Nurse Case Manager RGN.

• Salary £40,000 - £45,000 per annum (DOE)
• Mileage reimbursed at 45p per mile

• Full UK driving licence and access to a vehicle - essential
