About the role
As the MiCare Nurse under the direction of the Assistant Team Manager, you will enhance health and social care support for MiCare service users in line with Rutland County Council policies, objectives, and standards. This role is central to promoting independence, enabling choice and control, and preventing unnecessary hospital or care home admissions.
Key Responsibilities include
- Support the development of a neighbourhood and prevention agenda through a strong multi-disciplinary team (MDT) approach.
- Provide clinical advice and support to the MiCare Team as required.
- Deliver training on delegated healthcare tasks to staff.
- Build effective links with GP surgeries and community nursing teams to ensure a holistic approach for individuals using the service.
- Contribute to care planning that prioritises independence and well‑being.

Community Support Services – MiCare is a short‑term integrated health and social care service that provides short‑term support to people in their own homes. We put people at the heart of the service and deliver exceptionally responsive, person‑centred care which enables individuals to live a life of their choosing. Person Specification Experience- Registered Nurse Level 1 (Adult) or Student Nurse due to qualify in the next 4 months
- Hold/be working towards an NMC approved qualification or have a planned start date for this.
- Experience of working within a nursing team – either as a nursing student or qualified nurse.
- Demonstrates effective communication and interpersonal skills.
- Demonstrates adaptability and flexibility dependent on work priorities.
- Demonstrates a strong interest in providing patient centred care and patient education.
- Understanding of confidentiality and data protection.
- Understanding of the service and the different pathways – Reablement, Safety Net End of Life, Crisis Response, Discharge to Assess, Community to Assess Complex.
- Registered Nurse Level 1 (Adult) or Student Nurse due to qualify in the next 4 months.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
