Position: Field Nurse Case Manager (GTA)

We are looking for an engaged individual to take on the role of Field Nurse Case Manager. This role is responsible for undertaking the management of specific cases that are assigned to them in regards to patient reimbursement and coordination for the Navie Go branch of the organization. The Nurse Case Manager is responsible for overall management of their patient cases and acts as a one point of contact to their assigned health care providers.
How you will make an impact
Be responsible for the entire coordination of the patient journey;Occasional travel for on-site clinic visits;Acting as the Primary point of contact for physicians to enroll patients into a Navie Go managed program;Establishing and maintaining a strong, trusting relationship with assigned clinics and physicians including site visits;Acting as the main resource for Navie Go to request missing or additional information from respective physician offices, pharmacies, and/or other Health Care providers;Coordinate and manage all aspects required in order to obtain prescription drug coverage for patients;Coordinate cases with insurance companies, physician' offices and healthcare professionals in order to assist a patient in their journey on treatment;Coordinate documents, submissions and all related paperwork to insurance companies and public funding programs;Coordinate clinic appointments and drug deliveries within timelines;Foster and build relationships with patients and key health care providers;Implement best practices as required in an effort to maximize a patient's access to drug coverage;Coordinate with pharmacy processes required once funding has been obtained;Apply an in-depth knowledge of public / private funding systems; andAble to work rotating shifts Monday through Friday, covering the hours of 8 AM to 8 PM EST.What you’ll bring to our team
Bilingualism - English/French - is an asset;Registered Nurse designation and current provincial licensing is required;Experience in the healthcare, pharmaceutical, or insurance industry is an asset;Minimum of 2 years’ experience in customer/client service and health care reimbursement experience;Strong proficiency in Microsoft and Google applications (Google apps, Word, Excel);Professional/courteous/efficient telephone manner;Proven ability to organize time, set priorities, and multi-task in order to meet various competing work deadlines;Strong communication and interpersonal skills;Ability to work independently and in a team environment;Ability to resolve issues effectively and efficiently;Critical thinking and problem solving abilities;Able to work rotating shifts Monday through Friday, supporting various regions of Canada.With one tree planted for every new hire, why not start your new job on a green foot and help us lower our carbon footprint.
