The Role
The Care Designer plays a critical leadership role in delivering exceptional client experiences while driving business results through caregiver leadership and operational excellence. This role requires someone who can “hustle with heart”—balancing compassion with accountability, autonomy, and results. Care Designers are self-led leaders who take ownership of outcomes, thrive in a collaborative environment, and are inspired by building meaningful relationships with seniors, families, caregivers, and community partners.
Key Responsibilities- Consult Conversion & Community Engagement
- Conduct Caring Consults, assessing clients’ physical, emotional, and environmental needs
- Develop and recommend personalized care solutions aligned with client goals
- Achieve a 90% consult conversion rate
- Act as a trusted advisor to clients and their families
- Build and maintain relationships with community leaders and professional referral partners
- Promote high-quality, innovative customer service and Nurse Next Door’s brand promise
- Attend all First Visits to ensure care plans and service agreements are executed properly
- Conduct supervisory visits to ensure caregiver fit and quality of care
- Develop, implement, and update individualized care plans
- Educate clients on self‑care and evaluate responses to care
- Monitor risks, identify trends, and adapt care as needed
- Coordinate care and evaluate outcomes based on client goals
- Deliver a premium client experience with a target Net Promoter Score (NPS) of 9+
- Lead, coach, and support the caregiving team to achieve results
- Set clear goals, provide ongoing feedback, and coach in the moment
- Recruit, train, and manage caregiver performance and availability
- Foster a continuous learning environment, including Discovery Day and monthly in‑service programs
- Conduct First and Supervisory Visits to evaluate caregiver performance and engagement
- Drive a positive employee experience with a target employee NPS of 9+
- Partner with the scheduling team to communicate changes in client needs and caregiver assignments
- Ensure consistent client coverage, including urgent care needs and caregiver cancellations
- Support scheduling as needed to meet operational requirements
- Maintain AMS (Attendance Monitoring System - Alaya Care) below 5%
- Ensure data accuracy and completeness in scheduling systems
- Maintain accurate and complete documentation, including care plans, client files, and employee records
- Conduct quarterly audits to ensure 100% accuracy and compliance
- Ensure compliance with provincial and federal legislation related to health, employment, and occupational health & safety
- Approve visits and premiums in preparation for payroll and billing cycles
Part‑time, primarily evenings and weekends; on‑call availability for evenings, weekends, and holidays; flexibility required (7 days per week availability); schedule provided 2 months in advance based on business needs.
Compensation$33.00 per hour
Reporting & Working Relationships- Reports to the Franchise Partner
- Leads the Caregiving Team and Care Ambassador Team
- Works closely with the Care Services Center
- Registered Practical Nurse (RPN) or Registered Nurse (RN) licensed to practice in Ontario
- Valid CPR and First Aid Certification
- Valid driver’s license and access to a reliable vehicle
- Vulnerable Sector Criminal Record Check (updated every 5 years)
- Negative TB test (updated every 5 years)
- Strong computer skills; experience with G‑Suite and electronic documentation systems (Alaya Care experience an asset)
- Exceptional written and verbal communication skills in English
- Strong organization, planning, time management, and multitasking abilities
- Demonstrated experience setting and achieving goals
- Customer service and leadership experience considered an asset
