This role involves working in a post-acute care setting, providing quality care to patients with chronic conditions such as dementia and diabetes. Key Responsibilities of the LPN/LVN

- Provide direct care to patients with chronic illnesses
- Monitor patient health and vital signs
- Administer IV medications as required
- Assist patients with activities of daily living (ADL)
- Document patient care using Point Click Care (PCC) system
