We are currently seeking GPs to join the Chapel Street Surgery team, part of Sussex Primary Care, a larger network that provides support, collaboration, and development opportunities.

Main DutiesAs a GP in our practice, you will be responsible for providing primary medical services to the registered patients of the practice, ensuring the highest standards of care for all, undertaken in line with the relevant national primary care contractual regulations and guidelines. Provide care for both on‑the‑day care for patients and pre‑booked patients with long‑term conditions and be responsible for maintaining the smooth running of the practice.

Chapel Street Surgery is situated in Newhaven a port town in East Sussex, and we have approximately 5,000 patients. We provide a wide range of services including well woman clinics, contraception, maternity and midwifery, asthma / COPD and diabetes clinics, hypertension clinics, childhood immunisations, and minor surgery. We have a multi‑disciplinary team including GPs, Paramedic Practitioner, Physiotherapist, Pharmacy Team, Women’s Health Nurse, and Advanced Nurse Practitioners.
Chapel Street Surgery is a part of Sussex Primary Care (SPC), a primary care Federation that came about in response to a need from GP Partners in local practices who were seeking a different future. In 2019 we took on our first two practices and now have seven practices reflecting a combined list of over 50,000 patients.

- Surgery consultations and emergencies, telephone consultations and queries, triaging, visiting patients at home, checking and authorising repeat prescriptions and dealing with queries, patient test results, patient medical reports and examinations on behalf of the Practice as well as referral letters and other paperwork and correspondence in a timely manner.
- Provide health promotion and advice, signposting patients to appropriate services to maximise their health and social functioning and their ability to self‑manage their care.
- Be constantly mindful of the need for the provision of personalised care and population health, including reducing health inequalities and ensuring that these are reflected in daily practice.
- Make professional, autonomous decisions in relation to presenting patients, whether self‑referenced or referred from other health‑care workers within the organisation or external organisations.
- Assess the health needs of patients with undifferentiated and undiagnosed problems taking appropriate action as necessary.
- Screen patients for disease risk factors and early signs of illness taking appropriate action as necessary.
- Consult with patients and in line with current disease‑management protocols and care pathways, develop care plans for health‑related illnesses and share these in line with patient need and good information governance.
