We are seeking an enthusiastic community Assistant Practitioner to join our highly motivated, innovative team of Social Workers, Occupational Therapists and Assistant Practitioners. As an Assistant Practitioner within this team, you will undertake assessments, service commissioning and reassessment work. These will be carried out via video/telephone and/or face to face. You will have excellent communication, record‑keeping and keyboard skills, along with experience of working with vulnerable people. You should have a commitment to excellent customer service and be familiar with the Department's eligibility criteria.
In return, we can offer you the opportunity to work in an exciting and challenging area of Social Care, where you will be supported to develop into the role with regular supervision, training and peer support.

All our office‑based roles are currently working from home as part of our Smarter Working plans and to support the health and safety of all our colleagues. Working remotely will be a key feature of this role and when offices re‑open your office base will be in Dereham Hospital. We ask that you have good broadband connectivity.
