Care Coordinator (Patient Care Associate)

We’re hiring a full-time remote Care Coordinator to be the first voice a patient – or their family – hears when reaching out to Sailor Health.

In this role, you’ll guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.

This role is a unique blend of patient support, care coordination, and emotional guidance.

• Empathetic and clear communicator – You speak with kindness, listen actively, and make others feel at ease even during difficult conversations.
• Highly organized and detail‑oriented – You juggle multiple patients at different stages and never let things slip through the cracks.
• Tech‑comfortable – You’re comfortable learning tools like Google Workspace, Healthie, or Hub Spot.
• Mission‑aligned – You care deeply about improving care for older adults and are excited to play a key role in someone’s first step toward better mental health.
• Experience welcome, not required – You may have worked in healthcare, customer service, or administrative support before, but most importantly you’re organized, compassionate, and eager to grow.
