Are you a Pharmacist with extensive experience in Primary Care looking for a new challenge in your career?
We are seeking an enthusiastic and dedicated Primary Care Pharmacist to join One Medical Group and support our practice remotely. You will work mainly for Minehead Medical Practice in Somerset. This is an exciting opportunity to be an integral part of our healthcare team, offering your expertise in medicines management and pharmaceutical care to improve patient outcomes.
With extensive experience of working within a primary care setting, the post holder will provide expert support to our GP practice; they will be an essential part of our clinical team, contributing to the safe and effective use of medicines and optimisation of patient care. The Pharmacist will efficiently work as an expert advisor for medicines, repeat prescriptions, medicines optimisation and pharmaceutical care aspects of the multidisciplinary team workload, as well as support clinical decision-making for prescribing and treatment plans.
This is a full-time job (40 hours per week). However, part-time can also be considered
The job holder's key responsibilities will be
- Patient Care: Provide remote services to registered patients, offering advice on medicines, repeat prescriptions, and general pharmaceutical queries.
- Independent Decision Making: Having the confidence, skills, and judgement to take full professional responsibility for all pharmacy operations and patient care
- Remote consultation: Providing virtual patient consultations (via phone or video), including medication reviews, medicines optimisation, and patient education.
- Prescribing Support:
Assist with prescribing, audits, and managing high-risk drugs to ensure safe, effective patient care. - Patient Assessment: Assess patients’ knowledge and socio-economic factors, tailoring advice to meet individual needs.
- Prescription Management: Prescribe urgent medications using summary care records to ensure accurate re-prescribing.
- Clinical
Risk Management:
Identify and manage clinical risks and health queries, ensuring patient safety and optimal care. - Child & Adult Protection: Act on any child or adult protection concerns arising from health and medication calls, following set protocols.
- Collaborative Work: Work alongside multidisciplinary teams (MDT) to support service delivery and engage in quality improvement initiatives.
- Clinical System Documentation: Record accurate, timely information and advice within clinical systems and electronic databases.
- Regulatory Adherence: Uphold General Pharmaceutical Council standards of ethics, conduct, and performance in all aspects of practice.
- Professional Development: Participate in continuous professional development, working with colleagues to improve services and patient care.
What we are expecting from the post-holder:
- Registered with the General Pharmaceutical Council (GPhC).
- Completed the CPPE Pathway or due to complete it this year
- Non-Medical Prescribing qualification.
- Extensive experience working in a primary care setting.
- Experience in solo working and remote consultation.
- Proven ability to provide care through remote consultations and face-to-face interactions.
- Ability to manage workload effectively within a clinical service.
- Strong clinical expertise in medicines management and repeat prescription management.
- Excellent critical thinking, communication, and time management skills.
- Awareness of high-risk drug management to ensure safe prescribing practices.
- Knowledge of child and adult protection protocols.
- Competence in using clinical systems and maintaining accurate records.
