Ideally suited for a Level 3, social work qualified individual, especially if you have experience in hospital discharge and reablement.

Your role will be pivotal, as it involves local visits to Leicester residents’ homes, hospitals, and Discharge to Assess facilities across the city.

• Monitor and review clients’ progress, adapting care plans as necessary to meet their evolving needs.
• Maintain accurate and timely records of all interactions and interventions in line with the city’s policies and procedures.
• Provide expert advice and support to clients and their families, helping them navigate the healthcare system effectively.
