Social Worker – Reablement Team – Leicester – £34 Hourly – Ongoing

Join the dynamic Reablement Team in Leicester as a Social Worker, where you can shape the future of social care and make a significant impact through your expertise. This ongoing opportunity offers an hourly rate of £34 with flexible work arrangements, requiring two office days a week and the remaining days from home. Ideally suited for a Level 3, social work qualified individual, especially if you have experience in hospital discharge and reablement.

Your role will be pivotal, as it involves local visits to Leicester residents’ homes, hospitals, and Discharge to Assess facilities across the city.

What you will do
  • Collaborate with multidisciplinary teams to ensure effective discharge planning and reablement for clients.
  • Conduct assessments in clients’ homes, hospitals, and city facilities to tailor reablement programmes that promote independence.
  • Develop and implement personalised care plans in partnership with clients, family members, and healthcare professionals.
  • Monitor and review clients’ progress, adapting care plans as necessary to meet their evolving needs.
  • Maintain accurate and timely records of all interactions and interventions in line with the city’s policies and procedures.
  • Provide expert advice and support to clients and their families, helping them navigate the healthcare system effectively.
