Deliver high-quality physiotherapy across all pathways of Intermediate Care
, including reablement
, rapid response
, and rehab beds
.

Work collaboratively within multidisciplinary teams to ensure effective, holistic patient care.

Utilise post-graduate experience to manage complex caseloads
, providing tailored support for each patient’s individual needs.

Conduct detailed patient assessments and create personalised treatment plans to achieve the best rehabilitation outcomes.

Apply your community experience to deliver home-based interventions that promote independence and improve patients’ quality of life.

Minimum of two years’ postgraduate experience
, ideally within a multidisciplinary or rehabilitation setting.

A valid driving licence is advantageous for community visits.
