Job SummaryThe post holder is an experienced autonomous clinician who works within professional boundaries to provide care for presenting patients in the UTC. Responsibilities include initial history taking, clinical assessment, differential diagnoses, interpreting investigations where required, independent prescribing, and planning treatment with the patient; this may include self‑care or referral to specialty teams for both adult and paediatric patients.
The role will initially be face‑to‑face within the UTC, with opportunities for safe clinical decision‑making and expert care over the telephone, working from home taking NHS 111 Direct booking calls for UTC streamed patients. Shift patterns include weekends and night duties as the service operates 24‑hours.
Prospective candidates may progress to becoming an Advanced Clinical Practitioner following the 4 pillars of advanced practice.
Key Responsibilities- Use holistic patient assessment skills for adults and children to ensure prompt and appropriate action.
- Undertake assessment, diagnose, treat, advise and discharge or refer patients according to protocols.
- Request and interpret X‑rays and blood tests where clinically appropriate.
- Prescribe medications under own professional scope of practice and within the formulary, acknowledging professional boundaries.
- Keep accurate and immediate records of each enquiry including assessment and advice using CRS and Adastra.
- Encourage self‑care where appropriate through effective communication and health‑care advice.
- Act as an effective role model and advisor to the wider MDT.
- Work under pressure in a rapidly changing environment.
- Maintain confidentiality at all times, including storage and transfer of records.
- Promote good communication within multidisciplinary teams, with Trust and hospital staff, statutory and voluntary agencies.
- Assist patients and carers who face language barriers and take into account cultural beliefs and values.
- Identify safeguarding issues and act on the information.
- Support recruitment and selection processes, new staff orientation, and day‑to‑day team management.
- Contribute to the development, monitoring, auditing, and reviewing of policies, procedures, protocols, and guidelines within the UTC.
Maintain professional development by identifying training needs and attending relevant courses to ensure best practice. Keep up to date with current developments in patient care and evidence‑based practice. Work within all policies, protocols, and guidelines of the UTC and THGPCG.
Experience- Previous experience in an Emergency Care / Acute setting.
- Substantial post‑registration experience.
- NMC or HCP registration with no cautions or conditions of practice.
- Advanced Physical Assessment & Minor Injury Assessment qualification at Level 7.
- Independent non‑medical prescribing qualification.
