This is an excellent opportunity to join an innovative, compassionate, and collaborative Home team. The Public Health Improvement Practitioner post is a community based position within the Mid & Upper Nithsdale Home Team which works to improve the health and wellbeing of the local population. The practitioner is responsive to the needs, interests and circumstances of local people in the local community, helping individuals and communities to take more control over decisions that affect them and promoting health equity, equality, and justice.

Within the role, you will be working alongside colleagues to:

  • Manage the delivery and evaluation of specific programmes of work developed to improve the health and wellbeing of the local population.
  • Contribute to the development and delivery of the Community Health and Social Care Priorities and the Public Health Priorities for Scotland, and have a key role in influencing community plans.
  • Provide line management and/or support and supervision for staff within the team providing community‑based support services.
  • Develop and participate in a range of forums or partnerships with communities, agencies, and interdisciplinary groups to achieve shared approaches, opportunities, and outcomes for health improvement.
  • Provide leadership and influence in terms of evidence‑based community development approaches, community engagement, and health and wellbeing practice and principles.

This post will involve home visits in remote and rural areas. A current driving licence is an essential requirement of this post. However, if you have a disability that precludes you from holding a driving licence, consideration will be given if alternative transport can be arranged. Please detail how you would facilitate travel to clinics and home visits in your application.
