D2A Occupational Therapist

Slough are looking to recruit an enthusiastic

D2A Occupational Therapist to join a well-structured and dynamic team. The client offers a full time contract with an opportunity to work from home, a fair and manageable caseload and further training in your portfolio. The vacancy will be for an initial 3 - 6 months with potential for extension.

• Undertake strengths‑based occupational therapy assessments under the Discharge to Assess (D2A) pathway
• Support safe and timely discharge from hospital
• Assess functional ability, risk, and environmental factors in community settings
• Recommend and arrange equipment, adaptations, reablement, or short‑term support
• Work collaboratively with health partners, reablement teams, and social care colleagues
• Contribute to decision‑making around longer‑term care and support planning
• Maintain accurate records in line with statutory and professional requirements

• Previous experience working as an Occupational Therapist in the community
• HCPC Registration
• Enhanced DBS/CRB (we can obtain for you)
• Eligibility to work in the UK
• Occupational Therapist Qualification
