We are recruiting on behalf of our client, a leading care provider specialising in complex and community-based care services, for an experienced and compassionate Case Manager.
This role offers the opportunity to oversee and coordinate high-quality care packages for individuals with complex clinical needs, while working within a supportive and well-established organisation.
About the Role
In this role, you as a Case Manager, will take ownership of the transition, setup, and ongoing management of complex care packages, supporting clients with conditions including spinal injuries, acquired brain injuries, motor neurone disease, and multiple sclerosis. You will work closely with clients, families, and multidisciplinary professionals to ensure care is safe, effective, and truly person-centred.
The role combines home-based working with regular visits to client locations across Blackpool, Preston, and Carlisle, as well as weekly attendance at the Merseyside office. You will play a key role in care planning, risk management, staff support, and quality assurance, ensuring services consistently meet regulatory standards and best practice.
Key Responsibilities
* Oversee the assessment, transition, and ongoing management of complex care packages.
* Develop and review detailed care plans and risk assessments tailored to individual client needs.
* Work collaboratively with multidisciplinary teams to ensure high-quality, person-centred care delivery.
* Provide leadership, guidance, and mentoring to care and clinical staff.
* Monitor care quality, address concerns, and manage complaints in line with organisational policies.
* Lead on safeguarding matters and ensure compliance with CQC standards and regulatory requirements.
* Build strong relationships with clients, families, and external professionals to support positive outcomes.
About You
* A genuine passion for putting people first, with strong organisational and time management skills.
* Proven experience managing complex care packages involving clinical and neurological conditions.
* Strong understanding of CQC requirements, compliance standards, and safeguarding processes.
* Experience leading, supporting, or coordinating teams of healthcare professionals.
* Confident working independently while managing multiple priorities in a dynamic environment.
* Full UK driving licence and access to a vehicle for work purposes.
