General Practice Clinical Pharmacist

Overview

Pharmacists, are you ready for a change or to try something new? The Community Health and Social Care (CHSC) Pharmacy team is looking to recruit a motivated, proactive and enthusiastic pharmacist to work within our Annandale & Eskdale Cluster. Our team of pharmacists, pharmacy technicians and pharmacy support workers make up the GP practice clinical pharmacy service, based in both our Hubs and the GP Practice.

The team takes a leading role in medicines management and medication review as part of the primary care multidisciplinary team. It is a rewarding role, helping patients in Dumfries and Galloway to safely manage their medicines.

You will provide a clinical pharmacy service directly to patients. Day‑to‑day responsibilities may include a varied clinic in practice requiring clinical assessment of patients, carrying out complex polypharmacy medication reviews with patients, assessing patients’ ability to manage their medicines (including in the patient’s home), dealing with medicines‑related queries, medicines reconciliation, and prescribing medication. Joint working with GP practice staff, the wider multidisciplinary team in Health & Social Care, Acute and Community Pharmacy is important to ensure the best outcomes for patients.
