Are you looking for a unique opportunity? We are looking for an experienced OT, PT or SLT to support us in a Clinical Triage role for the Community Neuro Rehab Team.
We are seeking a Band 6 Therapist to be part of our supportive and friendly multi‑disciplinary team covering Southwark & Lambeth. This is a full time, fixed‑term contract to cover a 6‑month maternity leave, but flexible & remote working arrangements will be considered.
This position provides an excellent opportunity to develop your advanced clinical reasoning skills, risk assessment and communication skills. You will be participating in a new operational model for the service, utilising your specialist skills to identify appropriate pathways and support MDT prioritisation of patients. You will also have the opportunity to support the team to clinically manage appropriate cases within your professional field.
You will work closely with MDT and admin colleagues as well as being able to remain part of your wider professional group. You will have access to regular supervision from our Clinical Specialist OT or Clinical Specialist SLT including opportunities to attend appropriate training.

Main duties of the jobYou will be an AHP Therapist with postgraduate experience working with adults with acquired neurological impairments (physical, sensory, cognitive and communication). The position will suit individuals keen to enhance their specialism in neurological rehabilitation through advanced clinical reasoning skills, MDT communication and support the team to clinically manage appropriate cases within your professional field.
Ideally you will have some community neurological rehabilitation experience.
About usWe have a strong team ethos of personal & professional development and we value sharing our knowledge & skills within the MDT. You will be well‑supported through regular & direct clinical supervision and appraisal from a Band 7 Therapist of your profession plus specialist support from a Band 8 Clinical Specialist Occupational Therapist or Clinical Specialist Speech Language Therapist. You will have access to a planned programme of in‑service training within the wider community neuro rehabilitation team.
You will also have access to the benefits of belonging to Guys and St Thomas' Foundation Trust and Kings Health Partners, an Academic Health Science Centre.
The post holder will provide a specialist clinical triage service as part of the Neuro Rehab Team and will work closely with team members to ensure a coordinated approach to client care. The service works across the two London boroughs of Southwark and Lambeth in both community and acute positions. It provides MDT neuro rehabilitation and condition management to patients living with both long‑term and acute neurological conditions such as Stroke, Traumatic Brain Injury, Parkinson’s Disease, Multiple Sclerosis, Motor Neuron Disease and Neuro‑Oncology diagnoses.
Intervention is person‑centred and goal‑oriented. The service uses a self‑management approach, encouraging patients to participate actively in their rehab. The post holder will independently and effectively manage daily triage responsibilities and support a clinical caseload where capacity allows. The post holder will use advanced clinical reasoning skills, risk assessment skills and excellent communication skills to triage and prioritise referrals to the service.
