The role offers a flexible, hybrid schedule with 2 days in the office and 2 days of remote telehealth, a supportive environment, and a commitment to work‑life balance.
Eligible candidates have Indiana licensure, DEA, 3+ years in primary care, and NP/PA certification;
Spanish is preferred but not required.
