Location: New York
- Provide comprehensive telehealth care for dementia and cognitive disorders.
- Work within a multidisciplinary team to deliver personalized treatment plans.
- Participate in on-call rotation and support CMS GUIDE initiatives.
Qualifications:
- Active NP or PA license with at least 2 years in geriatrics or neurology.
- Strong preference for experience in dementia and cognitive care.
- Proficiency with telehealth technologies and EHR documentation.
- Remote-first culture with home office technology provided.
