The role is remote with up to 40 hours per week, delivering nutrition education, SMART goals, and progress monitoring via telephonic and video visits.
You will perform nutrition assessments, develop care plans, and collaborate with the health care team to ensure high-quality outcomes while adhering to regulatory standards.
