Pay Range
RN NSWOC Rate
: $51.94 - $53.66
Grounded in our commitment to Client and Family Centred Care the key foci on the RN role is to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.
We are currently seeking a Part‑Time NSWOC Nurse for the Toronto Central region. This flexible, self‑scheduling position allows you to create a work routine that best suits you. This role involves providing care predominantly in clients’ homes, with occasional clinic settings. A valid G‑class driver’s license and access to a reliable vehicle are required.
The NSWOC program focuses on prevention and reduction in the incidence of chronic wounds and pressure injuries. A NSWOC provides specialized holistic assessment and management as an inter‑professional team member to meet the needs of individuals with ostomies, acute, and chronic wounds. Care will take place within the community, with referrals from Ontario Health at Home. #INVHANS
Key Areas of Accountability- Assess, Monitor, and Makes Care Planning/Care coordination
- Evaluates care plan interventions through on‑going reassessment and evaluation results.
- Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
- Demonstrates professionalism, leadership, judgement, and accountability in interactions with client and families.
- Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
- Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
- Maintains documentation standards according to the College of Nurses of Ontario.
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- Must have completed the Wound, Ostomy and Continence Education Program (WOC‑EP) from NSWOCC.
- Preference will be given to candidates who have additionally completed the following: (1) a master’s program in Advanced Wound Care (MScCH WPC) from UofT, or (2) a Master of Clinical Science (MCISc) in Wound Healing Program from Western University or (3) an education program with the International Interdisciplinary Wound Care Course (IIWCC) from UofT.
- Membership of your Professional Association preferred (RNAO).
- RN diploma/degree from a recognized university or college.
- Hold a valid First Aid or CPR Certification.
- Hold a valid G‑class driver’s license and a reliable vehicle for traveling are required.
- Acute Care and Medical‑Surgical nursing experience is an asset.
- Knowledge of community nursing is an asset.
- Demonstrate effective communication (written and verbal) and documentation skills.
