Overview
We are looking for a
Nurse Specialized in Wound, Ostomy & Continence (NSWOC). The NSWOC will provide wound and ostomy consultation for patients in the clinic and community. If you enjoy caring for a broad range of clients with a variety of wound etiologies and wound care modalities this is the job for you. In addition to make full use of your wound and ostomy care knowledge, each day you will use your skills in promoting self-management by patients and leading the interdisciplinary team to achieve optimal patient outcomes.
Your expertise in the management of wounds and ostomies will be sought out by individual practitioners and the various committees and organizations that guide wound care in the region.
Care Partners will provide you with an in-depth orientation to both the agency and wound/ostomy care in the community; ongoing clinical support including online access to clinical expertise; an iPad and the equipment you need to conduct assessments.
We are looking for
Nurses with NSWOC/IIWCC certification

This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RN per ET visit range is $68.58 to $88.66 and the RN NSWOC hourly range is $42.50 to $51.47. General or non-NWSOCrelated visits are compensated at our RN rates. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate's knowledge, skills, qualifications, and relevant experience.What The Role Involves
Using your knowledge of assessment, prevention and interventions regarding wound infections and ostomy careCare for patients with consuming wounds, ostomy, continence, provide lower leg assessment including ABPIscritically appraising the wound care literature and apply research evidence to practiceProviding classifications and indications, appropriate utilization to reduce visit frequency and obtain desired outcomesPracticing conservative sharp wound debridement (CSWD) and use of other debridement methodsProviding edema management including bandaging based on complete lower leg assessmentMonitoring criteria for adjunctive therapy for complex or recalcitrant woundsSupporting our patient's safety by practicing according to policy and standardsUsing your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the Ontario Health atHome referral and other sourcesFunctioning as a member of a nursing team who are accountable for the nursing care it providesDeveloping appropriate time-specific, measurable goals and outcomes in collaboration with the client and/or caregiver(s), and in keeping with the Ontario Health atHome goal for treatmentWhat You Bring
Registered Nurse (RN) degreeCurrent College of Nurses of Ontario (CNO) registration in good standingGraduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pendingIIWCC or WCS CertifiedA strong focus on overseeing complex wound assessmentsSeveral years of escalating experience managing wounds and ostomiesA strong desire and commitment to making a difference in the lives of our patientsExceptional interpersonal skillsA valid driver's license and vehicle as regular travel is requiredExcellent English verbal and written communication skillsCPR CertificationCurrent immunization record including 2 step TB testTo protect our patients, we require a current vulnerable sector check and a clear background checkCare Partners In Your Community
