About Community Health Centres (CHC):
CHCs offer clinic services, referral services, support groups, therapy and ambulatory nutrition care, and a full range of addiction and mental health services, to those living in the Downtown Eastside (DTES) area. Each clinic is staffed with Integrated Care Teams (ICTs) to care for people who experience barriers accessing services, have no primary care providers and are challeged by complex health concerns (including psycho-social, physical, mental health and addiction co-morbidities).
Each ICT provides care on-site and via outreach, with nurses, social workers, peer specialists, community liaision workers, family physicians, nurse practioners, and psychiatrists. ICTs are supported on-site by more specialized care providers: wound care clinicians, palliative care nurses, OTs, dieticians, and many other disciplines.
As a Licensed Practical Nurse on the Downtown Eastside, you will:
Work collaboratively across multiple sites, clinics, and services, or on outreach/home based visits within an integrated health service delivery model and interdisciplinary care team, to provide holistic care with primary care clients.Care for clients and families who are experiencing health challenges across the primary care continuum.Seek direction/guidance from a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), Nurse Practitioner (NP), or Physician (MD) in situations where client care needs are not stable or predictable.Work closely with the RN/RPN/NP/MD in direct care activities using a team-based care approach, such as assessing, planning, implementing, evaluating and documenting client care as part of the team and collaboratively with other Community Care Services.Establish, maintain and enhance therapeutic relationships based on respect.Support clients and their families to achieve and maintain optimal health.Qualifications
Education & Experience
Graduation from a recognized program for Practical Nurses.Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).One (1) year of recent related experience or completion of a full final clinical placement in a community setting, working as a Licensed Practical Nurse providing care through a holistic mental health, substance use and/or primary care lens, or an equivalent combination of education, training and experience.Valid BC Driver’s License preferred.Knowledge, Skills & Abilities
Comprehensive knowledge of other health care disciplines and their role in client care.Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative, creativity and professional judgment.Ability to assess clients at risk and knowledge of appropriate acts; . Mental Health Act, Adult Guardianship, Public Health Act as it relates to the rights and obligations of clients and staff.Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Indigenous people.Knowledge and experience in Trauma Informed Practice, Harm Reduction and Recovery Oriented Care.Knowledge and practice within the standards dictated by the BCCNM for Licensed Practical Nurses.Broad knowledge of working with clients and family’s across the life span.Broad knowledge of Community resources and polices.Demonstrated ability to apply decision-making and analytical skills.Demonstrated ability to work collaboratively as a member of an interdisciplinary team.Demonstrated ability to communicate effectively both verbally and in writing.Demonstrated ability to effectively deal with conflict situations.Demonstrated ability to plan, organize and prioritize work.Physical ability to perform the duties of the position.Knowledge and experience with basic computer programs (Word, Microsoft…
