Sentara Health is seeking an Integrated Case Manager, RN. This remote position requires in-person face-to-face assessments and travel to Roanoke and northern areas up to 60–90 minutes to meet members approximately 50–75% of the week.
You will develop, monitor, and revise the member care plan to optimize health across the continuum, perform telephonic assessments, and coordinate care with the health team. A Bachelor's degree and managed care experience are preferred.
